North Carolina Adult Day Services Association Application For

| Our new rates include membership in the
National Adult Day Services Association, in addition to our other membership
benefits. Center membership fees are based on certified occupancy and
varies according to size of your center. Business or associate
memberships are also available and range from $150-$200 per year.
